Accepting New patients under the age of 41
All new patients must complete and submit the new patient paperwork. Once received by the office, the office will reach out to schedule the initial visit. All adult new initial visits are via televisit. Healthy pediatric children initial visit will be in office. All new potential pediatric patients must submit a vaccine record and any previous pediatric records if previously seen the in the US. We do not except emailed forms due to the sensitive nature of the record. Forms can be dropped off, mailed or faxed to 617-232-8086

For all sick visit requests please contact the office first thing in the morning, We strive to fit in our sick patients within 24 to 48 business hours.
Medication refill requests: Non-controlled can take up to two to three business days, controlled substances can take up to five business days.
Insurance referral requests may take up to three business days. When requesting an insurance referral please be sure to have the specialist full name, Fax number and NPI number.
Administrative Forms such as school and camp forms may take up to seven business days.
